Winter Concussion Testing Dates

Students in Athletic Activities must have a sports physical on file in the activities office to participate.  
 
Sports physicals are good for three years from the date of the exam.
( A well-child check does NOT qualify as a sports physical )
 
A link to the 2024-2025 MSHSL Sports Physical Form has been posted to the Activities Website under
the Activities Registration Tab. Please print this form for your doctor to complete.
 
Once the physical form is completed by your doctor, please email a copy to Taylor Roles,
DC Activities Secretary, at taylor.roles@dc.k12.mn.us and keep the original for your records.
